Personal information we collect from you
For OT assessments we collect:
- Name/Gender/Date of birth/Contact details
- Medical conditions relevant to the assessment
- Details may be collected by phone, email or through our contact form on our website
- If relevant to the product or service being provided, we may also collect information about
your mental health and emergency contact details. - We also collect information about your visit to our websites and the devices you use to access
them (desktop and mobile), and this may include an IP address. This information is used to
help us follow browsing preferences on our websites so we can improve them and enable
more advanced features to enhance the user experience. This is done through the use of google analytics.
How and why do we use your personal data ?
- We use the information we collect so we can assess you correctly for the appropriate equipment/services
- So we can let the relevant charity/benevolent fund know the outcome of an assessment so they can decide to support a request for financial assistance toward equipment/services relevant to your situation and let your caseworker (if relevant to your case) know the outcome.
- Legal Obligation – where there is statutory or other legal requirement to share the
information, for example, when we have to share your information with a regulatory body or
for law enforcement purposes.
• Vital Interests – where we need to process the personal data to protect someone’s life.
• Public task – where we need to process personal data ‘in the exercise of official authority’
(this covers public functions and powers that are set out in law); or to perform a specific
task in the public interest that is set out in law.
How will we store your personal information ?
- Written reports are stored in a locked filing cabinet with access only through the key holder Anne Ireland (Occupational Therapist)
- Any and all personal data held electronically will be and is password protected
- Only the relevant people/therapists/manager are allowed to access any personal information
- Erasing your information: When we have finished working together, we will erase electronic copies of your information & correspondence within 6 months. We will hold onto your written information for up to seven years (in line with guidance from the Health and Care Professions Council). After this time has passed, we will shred the written information.
How we may share your personal information?
- The results of the assessment will be shared (if relevant) with the charity and relevant parties such as equipment providers and the NHS, if the client gives their consent.
- If an individual case , information will only be shared with the client and it will be up to them to share it with other relevant parties.
Your rights
Under GDPR you have the right to ask for your information to be amended or deleted

- To be informed what information we hold (i.e. this document).
- To see the information we hold about you (free of charge for the initial request).
- To rectify any inaccurate or incomplete personal information.
- To withdraw consent to us using your personal information.
- To request your personal information be erased (though we can decline whilst the information is needed for us to practice lawfully & competently).
Cookie Notice
We collect information about your computer, including (where available) your IP address,
operating system and browser type, for system administration. This is statistical data about
our users’ browsing actions and patterns, and we will not use it in isolation to identify you or
any individual.
The OT Action Limited website is based on a content management system and,for
